Prescribing Notes:
- Epoetin may be used in patients with advanced chronic kidney disease (CKD) including those on haemodialysis, peritoneal dialysis or with a renal transplant. Darbepoetin alfa may be prescribed as an alternative to epoetin for those who would benefit from its once weekly administration instead of more frequent injections required with epoetin. Methoxy polyethylene glycol-epoetin beta may be prescribed in patients with pre-dialysis CKD would benefit from its fortnightly administration to reduce injection frequency.
- These drugs are initiated by specialists in secondary care but are appropriate for a shared care arrangements to facilitate the seamless transfer of individual patient care from secondary to primary care. Please refer to local shared care protocols for prescribing and monitoring advice.

Prescribing Notes:
- Epoetin for patients on peritoneal dialysis or home haemodialysis, patients with severe chronic renal failure approaching end stage, and renal transplant patients with failing grafts is appropriate for shared care arrangements to facilitate the seamless transfer of individual patient care from secondary care to general practice.
- Darbepoetin alfa may be prescribed for patients over 11 years as an alternative to epoetin and for those who would benefit from its once weekly administration instead of the more frequent injections necessary with epoetin.
